Caring for People Services is seeking a dedicated and knowledgeable Client Care Coordinator to enhance our team. The ideal candidate will play a crucial role in delivering extensive care management services to our clients. This includes evaluating needs, crafting personalized care strategies, coordinating services, and advocating for clients. The Client Care Coordinator will collaborate closely with clients and their families to create and execute tailored care plans that align with individual needs and aspirations.

Key Responsibilities:

  • Support clients and their families throughout the care process.
  • Evaluate client needs and formulate care plans accordingly.
  • Track client progress and modify care plans as necessary.
  • Establish and nurture partnerships with community organizations and professionals to guarantee optimal client care.
  • Educate clients and their families on available resources and services.
  • Ensure adherence to state and federal regulations regarding client care.
  • Maintain accurate documentation of all client interactions in a timely manner.
  • Fluently communicate with both Spanish and English-speaking clients.

Work Environment:

This position is classified as "Hybrid," indicating that daily responsibilities will be conducted partly from a designated office and primarily in the field.
