Primary Responsibilities:

  • Develop and implement comprehensive plans tailored to each client's unique needs and goals.\
  • Foster positive relationships with clients, their families, and multidisciplinary teams, ensuring seamless coordination and collaboration.\
  • Conduct regular assessments to monitor clients' progress, adjusting support strategies as needed to ensure optimal outcomes.\
  • Promote clients' independence and self-advocacy through education, empowerment, and advocacy.\
  • Maintain accurate records, adhering to HIPAA guidelines and agency policies while ensuring confidentiality and discretion.\
  • Cultivate strong communication and teamwork skills, collaborating effectively with colleagues, supervisors, and external partners.\\/ul\widgets{ul}
Requirements:
  • • High school diploma or equivalent required.
  • • Previous experience working with intellectually disabled clients or in a related field is highly desirable.
  • • Ability to pass a background check and obtain necessary certifications (CPR, First Aid, CPI) is mandatory.
  • • Strong analytical and problem-solving skills, combined with excellent communication and interpersonal abilities.
  • • Familiarity with relevant laws, regulations, and industry standards is beneficial.
