Job Summary

The Care Coordination Specialist will be responsible for coordinating patient care services, attending meetings as required, and following reporting guidelines. You will work closely with our medical director, attending physicians, and other healthcare professionals to ensure seamless care coordination.

Responsibilities
  • Coordinates patient care services, including hospice and long-term care services
  • Attends meetings as required and follows reporting guidelines
  • Communicates effectively with healthcare professionals and patients/families
  • Documents patient information accurately and timely
  • Participates in quality improvement initiatives to enhance patient care
Requirements
  • Registered Nurse with current license to practice in Indiana
  • Minimum of one year clinical nursing experience; hospice experience preferred
  • Demonstrated knowledge of regulatory requirements for hospice care
Salary and Benefits

We offer a competitive salary range of $65,000 - $85,000 per year, depending on experience. Our comprehensive benefits package includes health insurance, retirement plan, and paid time off.
