Job Summary

We are seeking a highly skilled and experienced Senior Patient Care Manager to join our team in Burlington, NC. As a key member of our leadership team, you will be responsible for overseeing the day-to-day operations of our hospice site, ensuring the delivery of high-quality patient care, and managing a team of healthcare professionals.

Responsibilities
  • Oversee the clinical aspects of the hospice program, ensuring compliance with regulatory guidelines and industry standards.
  • Manage and direct the services and performance of assigned team members, including supervising, managing performance, and directing the work of specified team members.
  • Administer the site in the absence of the Executive Director or Administrator, ensuring continuity of care and operations.
  • Manage and oversee clinical and administrative functions, including recordkeeping and patient assessment.
  • Respond to phone referrals and inquiries in a timely, professional, and compassionate manner.
  • Participate in on-call nursing schedule and provide direct skilled bedside nursing care in patients' homes.
Requirements
  • Registered Nurse (RN) with ASN or BSN from an accredited school of nursing.
  • One year of experience as an RN, with at least one year of management experience and two or more years of experience in hospice or home health.
  • Current license to practice nursing in the state of requested employment.
  • CPR Certification.
  • Current automobile insurance and valid driver's license.

  • Sensitivity to feelings of the terminally ill and their families before and after patient's death.
  • Ability to demonstrate flexibility and perform a variety of tasks, often changing from one to another of a different nature without loss of efficiency or composure.
  • Effective communication and interpersonal skills, with the ability to influence people in their opinions, attitudes, or judgments about ideas or things.
