Responsibilities

 

At Enhabit, our Hospice RNs work in private homes, residential care facilities, nursing centers and other hospice care environments.  Responsibilities include:

  • Conduct comprehensive assessments of patients' physical, emotional, and spiritual needs
  • Develop and implement individualized Care Plans based on patient assessments in the EMR system within 48 hours
  • Administer medications and treatments as prescribed by physicians plus call medication into pharmacy
  • Monitor patients' condition and document changes in their health status
  • Collaborate with interdisciplinary teams to coordinate patient care
  • Provide education and support to patients and their families on end-of-life care options
  • Assist with pain management and symptom control
  • Offer emotional support to patients and their loved ones
  • Attend IDG meeting and trainings
  • Must input and complete visit notes timely
  • Perform services in accordance with the physician’s orders and the established plan of care
  • Ensure delivery of care is compliant with the federal and state regulations.
  • Must be capable of compassionate communication with patients and team, have keen observation skills, high ethical standards, and knowledge of when to alert doctors and others about changes in patients' conditions.

 


Qualifications
  • Must be a Registered Nurse (RN), graduate of an approved school of clinical education and licensed in the state where they currently practice
  • Must have at least one year experience as a Registered Nurse
  • Must have 1+ year of hospice nursing experience
  • Medicare hospice experience is preferred
  • Must have basic demonstrated technology skills, including operation of a mobile device
  • Must possess a valid state driver license
  • Must maintain automobile liability insurance as required by law
  • Must maintain dependable transportation in good working condition
  • Must be able to safely drive an automobile in all types of weather conditions
  • Must possess CPR certification for the healthcare provider
