Position Overview:
The Resource Nursing Specialist will conduct hospice admissions for new patients. These admissions take place in various settings, including private homes, assisted living facilities, and skilled nursing establishments.

This role is an integral part of the interdisciplinary team, providing insights on psychosocial evaluations, interventions, and knowledge of community resources.


Required Qualifications:
Current Registered Nurse (RN) licensure (in good standing) in the relevant state(s)

Ability to collaborate effectively within a team environment, working closely with nurses, social workers, chaplains, volunteers, and CNA leadership.

Engage with patients on a personal level while receiving robust support from a dedicated care team.

Capable of working autonomously on a daily basis, with responsibilities that include

tasks such as:

communication via phone, email, and written correspondence, attending meetings with various healthcare professionals, and understanding hospice regulations.

Strong time management skills to balance visits, documentation, and meetings (i

e:
managing scheduled visits, attending necessary meetings, and responding to urgent needs by prioritizing and addressing multiple concerns)

Ability to learn and utilize an electronic medical records system.

Must possess reliable transportation, a valid driver's license, and auto insurance to facilitate travel across the service area.
