Job Summary

We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Resource Nurse. In this role, you will be responsible for completing hospice admissions for new patients, working closely with our interdisciplinary team to provide expert care and support.

Key Responsibilities
  • Complete hospice admissions for new patients, including assessments and documentation
  • Collaborate with the interdisciplinary team to develop and implement patient care plans
  • Provide expert psych-social assessments, interventions, and knowledge of community resources
  • Work closely with patients and their families to ensure seamless care transitions
  • Manage and prioritize multiple tasks and responsibilities, including documentation and communication with healthcare professionals
Requirements
  • Current Registered Nurse (RN) licensure in good standing
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong time management and organizational skills
  • Valid driver's license and reliable transportation
