Job Summary

Grady Health System is seeking a skilled Admission Nurse to coordinate admissions, assess potential residents, and provide case management and discharge assistance planning. The ideal candidate will have strong communication and interpersonal skills, with the ability to work independently and as part of a team.

Key Responsibilities
  • Coordinate admissions and assess potential residents to determine appropriateness of admission
  • Provide case management and discharge assistance planning to ensure smooth transitions from hospitals and community settings
  • Develop and maintain relationships with hospitals, healthcare providers, and professionals to enhance nursing home referrals and placement
  • Market the facility to hospitals, healthcare providers, and professionals to encourage and develop current and future referrals
  • Communicate with interdisciplinary teams to ensure clinical and durable medical equipment needs are met for all new admissions and re-admissions
Requirements
  • Graduate from an accredited school of Nursing as a Licensed Practical Nurse
  • Current Georgia License as a Practical Nurse
  • Ability to work independently with limited supervision
  • Ability to travel to various hospitals, healthcare centers, and other locations to assess patients within a 50-mile radius of the facility
  • Ability to communicate with other healthcare professionals
  • Ability to work with various departments within a long-term care setting
Preferred Qualifications
  • 3-5 years of experience in a Long-Term Care setting
  • 1-2 years of Marketing experience
  • Competency in Bilingual language speaking and writing
  • Knowledge of Medicare, Medicaid, HMO, Managed Care, and private insurance payer sources reimbursement for skilled nursing facilities
