Certified Peer Support Specialist

Wisconsin Community Services is seeking a Certified Peer Support Specialist to join our Keys to Independence Program. As a Peer Support Specialist, you will provide supportive, recovery-oriented interaction and engagement to individuals enrolled in the program. Your role will be that of a coach or mentor, utilizing your lived recovery experiences to promote wellness, purpose in life, and relationships.
Key Responsibilities:
Provide a strength-based assessment of an individual's abilities, strengths, assets, and needs.
Support individuals in completing a Personal Recovery Crisis Prevention Plan and Individualized Served Plan.
Monitor ADLs and assist with completing tasks as needed.
Maintain accurate documentation in Clarity and case files as required.
Work with collateral and community contacts to promote continuity of care.
Requirements:
High school diploma or GED required.
Graduate of State of Wisconsin Peer Specialist Training.
Certified in Wisconsin as a Certified Peer Specialist or become certified within one year of hire.
Meet all WCS and contractor requirements for criminal and caregiver background, driver's record, and reference checks.
Working Conditions:
The work schedule will meet the needs of the individuals being served. Ability to be mobile in the community and work activities involve movement within the community. Requires an ability to go up and down stairs and lift/carry 20 pounds.
