Position Summary: The Home Health Aide plays a crucial role in providing essential personal and palliative care to individuals in their residences. Key Responsibilities:

  • Assist clients in maintaining their physical health by adhering to care plans established by registered nurses. This includes aiding with personal hygiene, ensuring a safe living environment, preparing nutritious meals, and supporting mobility.
  • Provide emotional and psychological support to clients and their families, fostering trust and confidentiality while promoting engagement in meaningful activities.
  • Document care activities in accordance with company policies and promptly report any changes in clients' conditions to the supervising nurse.
  • Collaborate with an interdisciplinary team to communicate effectively about patient care and address any concerns regarding pain or emotional distress.
  • Respect and support clients' cultural practices, ensuring that care is delivered with dignity and compassion while prioritizing safety and quality.

Qualifications:

  • Completion of a state-required background check.
  • Registration on the Nursing Assistant Registry for Minnesota and North Dakota.
  • Current Basic Life Support (BCLS) certification.
  • Possession of a valid driver's license and reliable transportation.
  • Demonstrated ability to establish positive relationships with clients, families, and agency personnel.
  • A genuine commitment to providing care for the sick and disabled in a home setting.
  • Successful completion of orientation and ongoing competency assessments.
  • Compassionate nature with a focus on comfort-oriented care.
  • Physical capability to perform job-related duties.
  • Proficient in reading, writing, and speaking English to communicate effectively with all stakeholders.

Reporting Structure: This position reports to the Regional Clinical Manager.
