About the Role

We are seeking a dedicated and compassionate Registered Nurse to join our team as a Hospice RN Case Manager. In this role, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Key Responsibilities
  • Provide direct patient care services to hospice patients and supportive care to the patient's family.
  • Act as primary liaison between physician, patient/family and hospice team.
  • Manage and collaborate with an interdisciplinary group to provide comfort, peace, and dignity.
  • Evaluate patient and family needs and assist in the preparation and execution of the plan of care for each patient.
Requirements
  • RN license in the state of requested employment (or eligibility to obtain).
  • 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology strongly preferred.
  • Current driver's license and automobile insurance.
  • Current CPR certification.
