Patient Advocate Specialist

What You'll DoAs a Proton Care Navigator, you will be responsible for coordinating care for patients who have completed treatment and are returning routinely for follow-up care. This involves scheduling follow up appointments, coordinating external appointments for imaging and other specialist follow up, and advocating for our post-treatment patients in team meetings and other communications with the clinical team.
Your Key Responsibilities:
- Schedule and coordinate follow-up appointments for patients who have completed treatment.
- Coordinate external appointments for imaging and other specialist follow-up as needed.
- Act as a liaison between patients and the clinical team to ensure seamless communication and coordination of care.
- Develop and maintain relationships with referring providers and other stakeholders to ensure high-quality patient care.
- Stay up-to-date on the latest developments in proton therapy and cancer care to provide expert guidance and support to patients and their families.
