Bilingual Community Health Worker

We are seeking a highly motivated and compassionate Bilingual Community Health Worker to join our team at Vibrant Health - Wyandotte Neighborhood Clinics. As a key member of our community outreach team, you will play a vital role in supporting patients in managing their health and navigating the healthcare system.
Key Responsibilities- Assist patients in the community through home visits, attending medical appointments, and accessing other community-based resources.
- Review referral forms, needs assessment screeners, or Electronic Medical Record data to determine appropriateness for participation.
- Complete documentation in the identified electronic health record of all telephone and face-to-face contacts with patients in a timely and professional manner.
- Enroll patients in needed programs and document in approved format in a timely manner.
- Collaborate with patients to set client-centered treatment goals and review progress regularly.
- Assist patients in scheduling medical, behavioral health, and social service appointments and attend appointments as needed or requested.
- Ensure patients' access to interpreters and translators for medical, behavioral health, and social service appointments.
- Assist patients in securing transportation to medical, behavioral health, and social service appointments.
- Help patients obtain necessary medications through patient assistance programs and other reduced-cost programs.
- Provide support and education to patients to assist them in adhering to medical recommendations and taking medications as prescribed.
- Provide regular and timely updates to medical providers, referral sources, and clinics on patients' progress to meet treatment goals and troubleshoot any problems or concerns.
- Complete all required documentation and data tracking in a detailed and timely manner in the electronic health record.
- Attend and actively participate in all trainings and meetings as requested.
- High school diploma or equivalent plus lived experience or close connection to the community being served.
- Ability to develop rapport with patients and their families, community, and other healthcare personnel.
- Professional and respectful demeanor.
- Ability to work successfully independently and within a team, shifting tasks, prioritizing tasks, problem-solving daily, and completing work within deadlines.
- Excellent customer service, helpfulness, and positive attitude.
- Superior organizational skills with attention to detail and ability to multi-task and meet deadlines.
- Ability to recognize, understand, and appreciate the value of cultural diversity.
- Ability to demonstrate social and cultural sensitivity to ethnically and economically diverse patients and staff.
- Experience and working knowledge of local community resources.
- Current drivers' license with an acceptable driving record and reliable transportation.
- Ability to travel to all parts of the facility and throughout the community.
- Current CPR certification, which can be provided through Vibrant Health if needed.
- Proficiency in use of personal computers and Microsoft Office (Outlook, Word, and Excel).
- High school diploma; some college preferred.
- Current CPR certification, maintained through employment.
- Two years relevant work experience, preferably in a medical setting.
- Proficiency in use of personal computers and Microsoft Office Suite (Word & Excel) required.
- Previous customer service experience.
- Two years preferred experience with an electronic health record system.
