Licensed Clinical Social Worker

About the Role:
We are seeking a Licensed Clinical Social Worker to join our team in Manchester, NH. As a Clinical Social Worker, you will provide counseling services to individuals with traumatic brain injuries, spinal cord injuries, or acute diagnoses. You will work in a home and community-based setting, providing therapeutic interventions and resource recommendations to support patients' recovery and independence.
Responsibilities:
- Perform comprehensive evaluations to assess patients' social, emotional, and support needs.
- Set measurable objectives in conjunction with the rehab team.
- Assist physicians and other health team members in understanding patients' social and emotional factors related to their health problems.
- Instruct and counsel patients and families in treating and coping with social and emotional responses to illnesses.
- Participate in care coordination activities and act as a resource to other health team members in identifying and resolving patient needs.
- Identify and assist patients and families in utilizing community resources to achieve measurable objectives.
Qualifications:
- Master's Degree in Clinical Social Work (LCSW).
- Current Licensed Clinical Social Worker licensure in the state of practice.
- Minimum of one year experience in a rehabilitation setting serving individuals with traumatic brain injuries, spinal cord injuries, or acute diagnoses.
- CPR certification required.
- Ability to lift 50 pounds and perform physical demands of the job.
