About the Role:

We are seeking a highly skilled Licensed Clinical Social Worker to join our team at Rehab Without Walls Neuro Rehabilitation. As a Clinical Social Worker, you will play a vital role in providing counseling services that support individuals with physical, mental, cognitive, and emotional adjustment through recovery.

Key Responsibilities:

  • Perform comprehensive evaluations to assess the social, emotional, and support needs of patients and their families.
  • Set measurable objectives in conjunction with the rehab team to promote patient recovery.
  • Assist physicians and other health team members in understanding the significant social and emotional factors related to patient health problems.
  • Instruct and counsel patients and families in treating and coping with social and emotional responses connected with illnesses.
  • Participate in care coordination activities and act as a resource to other health team members in identifying and resolving patient needs.
  • Identify and assist patients and families in utilizing appropriate community resources to achieve measurable objectives.

Requirements:

  • Master's Degree in Clinical Social Work (LCSW)
  • Current Licensed Clinical Social Worker licensure in the state of practice
  • Minimum of one year experience in a rehabilitation setting serving individuals with traumatic brain injury, spinal cord injury, or acute diagnosis
  • CPR certification required
  • Ability to lift 50 pounds and perform physical demands of the job
