Senior Care Clinic Nurse

We are seeking a compassionate and skilled Registered Nurse to join our Senior Care Clinic team at Harborview Medical Center. As a key member of our multidisciplinary team, you will provide high-quality care to patients aged 65 and older who require geriatric services.
Key Responsibilities:- Provide in-person and phone triage, in-basket management, and thorough symptom assessment to develop clinical interventions and plans of care.
- Collaborate with home health and community living providers to ensure seamless transitions of care.
- Address a range of patient concerns, including blood pressure management, edema management, and wound care.
- Active WA State or Multi-State RN License by start date.
- At least 2 years of professional acute care, med-surg, or primary care nursing experience.
