Senior Care Clinic Registered Nurse

We are seeking a compassionate and skilled Registered Nurse to join our Senior Care Clinic team at Harborview Medical Center. As a key member of our multidisciplinary team, you will provide high-quality patient care and work collaboratively with colleagues to deliver exceptional outcomes.
Responsibilities:- Provide in-person and phone triage, in-basket management, and perform thorough symptom assessments
- Develop clinical interventions and plans of care, and manage transitions of care
- Interact with home health and community living providers to address patient needs
- Active WA State or Multi-State RN License by start date
- At least 2 years of professional acute care, med surg, or primary care nursing experience
