Position Overview

The Case Manager plays a crucial role in aiding Ukrainian humanitarian parolees and other eligible immigrants, ensuring they are connected to vital resources and services. This position involves conducting assessments and developing tailored service plans to meet the unique needs of each client.

Key Responsibilities
  • Deliver exceptional service to clients, facilitating access to benefits and resources.
  • Conduct intake interviews and assessments to determine eligibility for programs, crafting individualized service plans.
  • Provide timely referrals for social services, health care, and legal assistance, based on client assessments.
  • Distribute direct assistance funds and track their usage, ensuring clients have the necessary tools to achieve their goals.
  • Regularly evaluate client progress, adjusting plans and documentation as needed to ensure compliance and effectiveness.
  • Engage in home visits and collaborate with community service providers to enhance client support.
  • Participate actively in team meetings and professional development opportunities.
  • Perform other related duties as assigned.

Qualifications

Education and Experience:
A Bachelor's degree in social work, human development, or a related field is required, along with a minimum of one year of experience working with refugees or immigrants, including case management experience.

Skills:
The ideal candidate will possess strong computer skills, particularly in MS Office Suite, and demonstrate exceptional organizational and time management abilities. Excellent communication skills are essential, and the ability to thrive in a multicultural environment is required.
