Job Title: Judicial Case Manager

Centennial Mental Health Center is seeking a highly skilled and compassionate Judicial Case Manager to join our team. As a key member of our staff, you will play a vital role in providing direct behavioral health and case management interventions to individuals involved in the criminal justice system.

Job Summary:

The Judicial Case Manager will work closely with clients, courts, and community agencies to provide comprehensive case management services. This includes developing and implementing individualized service plans, coordinating with treatment providers, and facilitating client engagement in treatment.

Key Responsibilities:
  • Provide supportive behavioral health interventions to clients in individual or group settings.
  • Complete brief screenings of clients referred through judicial entities and engage them in case management services as appropriate.
  • Develop, monitor, and follow up on client case management service plans.
  • Assist clients with resource acquisition, linkage to agency and community services, and behavioral health skill development.
  • Collaborate with treatment providers to ensure seamless service delivery and enhance treatment engagement and success.
  • Serve as the primary source of contact and information for referral parties, including local probation departments and courts.
  • Represent the agency in local problem-solving courts, if assigned.
  • Provide services in a variety of settings, including client homes, community locations, and Centennial offices.
  • Maintain accurate and timely clinical records consistent with Center standards.
  • Participate in efforts to collaborate with local jails to facilitate care coordination for individuals releasing from incarceration into Centennial communities.
  • Serve as a Case Management resource to individuals releasing from local jails, assisting with engagement into treatment and other practical needs during the transition phase.
  • Facilitate the DRS process for DUI clients, if assigned.
  • Respond to referrals for Outpatient Competency Restoration services in a timely manner.
  • Provide outpatient Restoration services to referred clients, if assigned.
  • Participate in meetings, training, and in-services.
  • Participate in supervision sessions.
  • Perform other job duties as assigned.
Requirements:
  • Bachelor's degree in psychology, social work, or other human services field required.
  • Experience working in behavioral health, human services, adult education, or rehabilitation preferred but not required.
  • Ability to work well with special populations and maintain appropriate boundaries.
  • Able to identify and nurture strengths and support client rights to make independent decisions.
  • Skills to serve culturally diverse populations that may have a bearing on service provision.
  • Effective written and verbal communication skills.
  • Demonstrates effective time management and the ability to multi-task.
  • Ability to work as a team member.
  • Knowledge of basic computer literacy, such as email communication and word processing.
  • Ability to travel within the Center's service area or to other locations as needed.
Salary:

$22.12/hr - $26.56/hr
