POSITION OVERVIEW

The Community Support Specialist is responsible for establishing new partnerships and nurturing existing relationships with community organizations, while also delivering crisis intervention services to youth facing challenging circumstances. This role focuses on providing immediate support and facilitating reunification for youth who are away from home without parental consent or in situations that pose a significant risk to their safety, aiming to divert them from entering the Juvenile Justice System.

KEY RESPONSIBILITIES

Crisis Intervention Responsibilities:

  1. Act as a responder for designated Local Area Networks (LAN) as needed, covering various counties.
  2. Deliver daytime crisis intervention and case management services to all youth and families without decline.
  3. Maintain adherence to the crisis response schedule, ensuring immediate availability via cell phone.
  4. Provide crisis intervention services to referring sources (hospitals, schools, police) within specified timeframes.
  5. Complete all necessary intake and crisis documentation, assessments, and service plans promptly.
  6. Coordinate with the Program Coordinator for authorization prior to youth placement.
  7. Facilitate reunification and arrange appropriate placements during crises, along with providing tailored referral services.
  8. Ensure all crisis intervention documentation is accurate and submitted timely.
  9. Enter participant data into the designated systems within required timeframes.
  10. Collaborate with Community Support Specialists for follow-up on youth in placements and provide updates as necessary.
  11. Offer transportation for clients in emergency placements to appointments and school as required.
Daily Responsibilities:
  1. Respond promptly to communications from the Program Coordinator.
  2. Keep the Outlook calendar updated with daily tasks and travel plans.
  3. Maintain and develop community relationships through regular outreach.
  4. Organize and participate in community outreach events.
  5. Provide necessary services to program clients during on-call shifts.
  6. Conduct life skills groups and manage an active caseload as assigned.
  7. Ensure client files are maintained and up-to-date.
  8. Monitor and supervise clients to ensure their safety during programming hours.
QUALIFICATIONS

To succeed in this role, candidates must demonstrate the ability to fulfill each essential duty effectively. The qualifications listed below are indicative of the knowledge, skills, and abilities required.

EDUCATION and EXPERIENCE

A Bachelor’s degree (BA/BS/BSW) is required, with prior experience working with at-risk youth preferred.

ADDITIONAL QUALIFICATIONS
  • Availability to work evenings and weekends with flexible scheduling.
  • Must have a valid driver's license and be able to transport clients in a personal vehicle.
  • Ability to accept and incorporate constructive feedback.
CERTIFICATIONS
  • Valid driver's license with a clean driving record.
  • Proof of current automobile insurance.
