About the Role

We are seeking a skilled School-Based Client Support Specialist to join our team at Health Connect America, Inc. in Walker County, GA.

Key Responsibilities:
  • Provide community-based support services for child, adolescent, or adult clients to help them achieve and maintain rehabilitative support.
  • Meet with clients to review mental health symptoms, assess needs, and provide interventions as outlined in the treatment plan.
  • Assist clients and their families in managing or preventing crisis situations by reviewing all assessments and notes related to the individual's history, diagnosis, and treatment plan.
  • Support clients in articulating and achieving their goals by identifying strengths and providing skills support.
Requirements:
  • A High School Diploma with 2 years' experience or a Bachelor's Degree in social work, psychology, or a mental health-related field.
  • Strong communication and interpersonal skills.

  • An estimated salary of $45,000 - $60,000 per year based on location and experience.
