Job Summary

We are seeking a highly motivated and compassionate Direct Support Professional to join our team at Family Options, Inc. As a Direct Support Professional, you will play a vital role in providing habilitative assistance and care to clients living in our community and supported living arrangements, family support services program, and day/vocational program.

Key Responsibilities
  • Provide direct daily care, teaching, and supervising of clients to include record keeping.
  • Assist in the preparation of individualized plans of service and implement goals and staff actions developed.
  • Teach, supervise, and assist clients in performing adult daily living activities, such as hygiene, personal care, eating, banking, shopping, meal-planning, clothing planning and care, community participation, self-control, etc.
  • Monitor client progress on goals and protect client rights.
  • Report incidents of abuse, neglect, and mistreatment.
  • Implement behavioral plans, including approved or emergency restraints and procedures when necessary.
  • Develop and implement recreation and community participation activities with all clients in your care.
  • Work cooperatively with Clinical Director, Consultants, families, schools, and other agencies in providing and implementing goals.
  • Maintain accurate and timely record keeping of services provided to clients and specific goals identified in each client's plan of service.
Requirements
  • High School Diploma or equivalent.
  • A minimum of two years experience working with individuals with developmental and/or behavioral challenges is preferred.
  • Successful completion of Medication Certification Program, PMT, CPR, and First Aid training program.
  • Valid CT driver's license with a satisfactory driving record.

The noise level in the work environment is usually moderate. While performing the duties of this job, the employee travels by automobile and is exposed to changing weather conditions.
