This role is essential in our mission to provide empathetic support to individuals with limited access to resources. The ideal candidate will demonstrate a commitment to social equity, uphold human dignity, and embody integrity while actively engaging with the community.
Key Responsibilities
- Guide and mentor individuals with intellectual and physical disabilities in social skills as per their personalized integration strategies.
- Provide training and support in daily living competencies, including social interactions, domestic tasks, community engagement, vocational skills, and personal care.
- Assist individuals in an independent living setting with meal preparation, personal hygiene, grocery shopping, household tasks, and medication management.
- Support individuals with comprehensive care related to bowel and bladder management.
- Deliver physical assistance and oversight aligned with the dietary requirements of individuals.
- Facilitate community integration for individuals while ensuring their health and safety.
- Maintain awareness of each individual's service needs and behaviors to aid in achieving personal objectives.
- Conduct daily documentation and data collection regarding the progress and setbacks of individual goals.
- Timely report any issues to supervisors to ensure prompt resolution.
- Establish and nurture collaborative relationships with community organizations, program supervisors, caregivers, colleagues, and family members.
- Advocate for and positively represent the program to funding bodies, community organizations, families, and relevant stakeholders.
- Provide guidance and redirection to individuals facing behavioral challenges during crisis situations.
- Document and report all incidents, accidents, and behavioral occurrences related to individuals.
- Adhere to all HIPAA regulations and maintain confidentiality of individual information at all times.
- Prepare summaries of clients' community progress and communicate any concerns to supervisors promptly.
- Participate in mandatory staff meetings on a monthly and quarterly basis.
- Complete necessary documentation following incidents or accidents and submit to supervisors.
- Comply with company policies and procedures while upholding Title 22 regulations.
- Exhibit professionalism in the workplace consistently.
- Submit timecards and expense reports to supervisors according to established timelines.
- Develop and implement lesson plans based on required curriculum outlines.
- Maintain a tidy working environment, including occasional cleaning of shared facilities.
- Follow the medical plans outlined by physicians for individuals.
- Ensure a safe environment for individuals, preventing harm to themselves and others.
- Foster a collaborative team atmosphere, supporting and executing project ideas.
- Maintain current CPR and First Aid certifications.
- Report safety concerns to management in a timely manner.
- Adhere to outlined program plans concerning behavioral interventions.
- Perform additional duties as assigned by supervisors.
- High School Diploma or Equivalent.
- CPR and First Aid certification required.
- CNA certification is a plus.
- Valid Class C Driving License.
- Familiarity with assistive devices (e.g., mechanical lifts, wheelchairs, walkers, hearing aids, adaptive feeding utensils) and proficiency in Microsoft Office Suite.
- Willingness to cross-train across departments to meet the diverse needs of individuals and effectively problem-solve during crises.
