Gentiva Hospice is seeking an experienced Hospice RN/RN Case Manager to join our team in Atlanta, Georgia.

About the Role

As a Hospice RN/RN Case Manager, you will provide direct patient care services, manage patient clinical records, and participate in quality assessment and performance improvement programs.

You will also be responsible for educating caregivers, managing an interdisciplinary team, and collaborating with physicians and other healthcare professionals.

Responsibilities and Requirements
  • Direct patient care services, including evaluating patient and family needs and assisting in the preparation and execution of the plan of care
  • Manage patient clinical records and participate in quality assessment and performance improvement programs
  • Educate caregivers and empower them to provide supportive care to patients
  • RN license in the state of employment (or eligibility to obtain)
  • 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology
  • Demonstrated flexibility, composure, and empathy when supporting patients and their families
Compensation and Benefits
  • Competitive salary: $105,000 per year
