Salary:

$67,900 - $83,800 per year, depending on experience

About the Role:

We are looking for a highly motivated individual to provide psychosocial support services to patients and families in our Neurology Partial Hospitalization Program. As a Psychosocial Support Specialist, you will work closely with our multidisciplinary team to develop and implement comprehensive treatment plans, provide education and resources to patients and families, and collaborate with healthcare professionals to ensure seamless care transitions.

Responsibilities:
  • Conduct thorough psychosocial assessments to identify patient and family needs
  • Develop and implement individualized treatment plans to address patient and family concerns
  • Provide education and resources to patients and families to promote healthy coping mechanisms and adherence to treatment plans
  • Collaborate with healthcare professionals to ensure effective communication and coordination of care
  • Participate in interdisciplinary team meetings to discuss patient progress and make adjustments to treatment plans as needed
Requirements:
  • Masters degree in Social Work (MSW) or related field
  • Licensed Social Worker (LSW) or eligible for licensure
  • 1-2 years of experience working in a mental health setting
  • Strong understanding of child and adolescent development, mental health diagnosis, and treatment planning
  • Excellent communication and interpersonal skills
