Job Summary

Families and Youth Inc is seeking a highly skilled Resource and Referral Specialist to join our team.

The successful candidate will be responsible for providing exceptional customer service, navigating the needs of individuals seeking services, and coordinating referrals to internal and external resources.

Key Responsibilities
  • Provide administrative support and triage customer needs to guide them to appropriate services
  • Assist customers in completing onboarding paperwork and applications for health care professionals and community services
  • Coordinate warm handoffs between providers and schedule new patients for intake assessments
  • Manage agency data tracking systems and NM Star database for referrals and client data
  • Verify private insurance and Medicaid eligibility for clients
  • Utilize EMR clinical database to activate new clients and manage referrals
Requirements
  • Bachelor's degree in a human service field and one year of relevant experience, or associate's degree with two years of experience
  • Pass background checks and maintain certifications in CPR/First Aid and CPI
