Senior Clinical Case Manager

The Manager for Case Management Services is responsible for promoting patient-centered care by coordinating all aspects of hospital-based case management. This role oversees the day-to-day management of the case management program, including coordination, supervision, and administrative oversight of the case management team. The Manager will function as an expert clinical practitioner, case management subject-matter expert, resource, advisor, and leader for the members of the case management team.
Key Responsibilities
Coordinate activities that promote quality outcomes and patient throughput
Supervise and monitor professional and support staff
Ensure effective care coordination and case management practices are consistent with hospital policies and applicable regulations and guidelines
Requirements
2-year / Associate's Degree in Nursing or 4-year / Bachelor's Degree in Nursing (2-year grace period) or Master's Degree in Social Work
3+ years experience in Acute-Care Case Management or Social Work
2+ years supervisory experience
