Job DescriptionThe Manager for Case Management Services plays a critical role in promoting patient-centered care by coordinating all aspects of hospital-based case management. This role is responsible for the overall day-to-day oversight and management of the case management program, including coordination, supervision, and administrative oversight of the case management team.
This position requires a strong clinical background, with experience in acute care case management or social work. The ideal candidate will have a minimum of 3 years of experience in case management, with 2 years of supervisory experience. A Bachelor's Degree in Nursing or a Master's Degree in Social Work is required.
The Manager for Case Management Services will be responsible for:
- Coordinating activities that promote quality outcomes and patient throughput
- Supervising and monitoring professional and support staff
- Ensuring effective care coordination and case management practices
- Monitoring quality of interactions, documentation, and assessments
