- 2-year/Associate's Degree in Nursing or 4-year/Bachelor's Degree in Nursing (with 2-year grace period) or Master's Degree in Social Work.
- 3 years' experience in Acute-Care Case Management or Social Work.
- 2 years' supervisory experience.
