Job Summary:

The RN Manager of Case Management is responsible for overseeing the day-to-day operations of the case management program, ensuring effective care coordination and case management practices are consistent with hospital policies and regulations. This role requires a strong clinical background, leadership skills, and the ability to supervise and monitor professional and support staff.

Key Responsibilities:

  • Coordinate and supervise the case management team to ensure quality outcomes and patient throughput.
  • Monitor and evaluate the quality of interactions, documentation, and assessments of the case management staff.
  • Develop and implement policies and procedures to ensure compliance with hospital regulations and guidelines.
  • Collaborate with other departments to ensure seamless care coordination and patient flow.
  • Provide education and training to staff on case management best practices and hospital policies.

Qualifications:

  • Associate's Degree in Nursing or Bachelor's Degree in Nursing (2-year grace period) OR Master's Degree in Social Work.
  • 3+ years' experience in Acute-Care Case Management or Social Work.
  • 2+ years supervisory experience.
