The Manager for Case Management Services plays a critical role in promoting patient-centered care by coordinating all aspects of hospital-based case management for their assigned area of responsibility. Key Responsibilities- Oversee the day-to-day management of the case management program, including coordination, supervision, and administrative oversight of the case management team.
- Function as an expert clinical practitioner, case management subject-matter expert, resource, advisor, and leader for the members of the case management team.
- Supervise and monitor professional and support staff to ensure effective care coordination and case management practices are consistent with hospital policies and applicable regulations and guidelines.
- Monitor the quality of interactions, documentation, and assessments of the case management staff.
- Coordinate activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization.
The ideal candidate will possess strong leadership and clinical skills, with the ability to coordinate and manage complex case management programs. A strong understanding of hospital policies and regulations is essential, as is the ability to work effectively with multidisciplinary teams.
