Position Overview:

CommonSpirit Health is dedicated to delivering exceptional healthcare services, and as part of this commitment, we are seeking a Transfer Center Coordinator. This role is vital in ensuring seamless communication and coordination between referring hospitals and our healthcare facilities.

Role Summary:

The Transfer Center Coordinator serves as the initial point of contact for hospitals and healthcare providers, offering essential information and operational support. Utilizing a comprehensive understanding of clinical software, this position provides crucial administrative assistance to the Transfer Center operations.

Key Responsibilities:

  • Facilitates the intake process for incoming transfer requests, ensuring effective support for transferring facilities.
  • Responds to incoming inquiries, addressing specific patient care requirements.
  • Generates Medical Record Numbers (MRN) and Financial Identification Numbers (FIN) for new patient admissions.
  • Maintains clear and efficient communication with Intake RNs, referring facilities, and hospital providers through verbal and electronic messaging.
  • Connects external referring providers with emergency department staff for smooth transitions of incoming emergency patients.
  • Oversees future date direct admissions, ensuring timely communication with facilities and patients.
  • Collaborates with central patient placement to facilitate prompt bed assignments.
  • Acts as a liaison for interfacility transfers, providing regular updates to sending facilities and documenting all interactions in the transfer software.
  • Completes transfer requests upon resolution and partners with central patient placement to enhance bed assignment efficiency.

Data Management:

Maintains a comprehensive database for stroke transfers and generates monthly reports as required.

Support Functions:

Assists the Transfer Center RN with transfer intakes as time allows.

Qualifications:

High School diploma or GED is required, along with a minimum of two years of experience in a healthcare setting. A solid understanding of medical terminology is essential.
