Role Summary:
The Transfer Center Coordinator serves as a vital point of contact for referring hospitals and healthcare providers, delivering essential information and operational support. This position requires a comprehensive understanding of clinical software to facilitate administrative tasks effectively. The Coordinator plays a crucial role in managing transfer operations, which includes updating patient demographics, generating medical record numbers, and ensuring seamless communication between referring providers and emergency department staff.
Key Responsibilities:
- Facilitates the intake process for incoming transfer requests while assisting transferring facilities.
- Responds to inquiries and provides support to transferring facilities based on specific patient care requirements.
- Generates Medical Record Numbers (MRN) and Financial Identification Numbers (FIN) for new patient admissions.
- Maintains effective communication with Intake RNs, referring facilities, and hospital providers through verbal and electronic messaging.
- Connects external referring providers with emergency department personnel for smooth transitions of incoming emergency patients.
- Oversees future date direct admissions, ensuring clear communication with facilities and patients.
- Collaborates with central patient placement to ensure timely bed assignments.
- Acts as a liaison for interfacility transfers, providing regular updates to sending facilities and documenting all interactions in the transfer software.
- Completes transfer requests upon finalization.
- Maintains the stroke transfers database and generates monthly reports as required.
- Assists the Transfer Center RN with intake processes as time allows.
Qualifications:
High School diploma or GED is required. A minimum of two years of experience in a healthcare setting is preferred, along with a solid understanding of medical terminology.
