As a key member of our Transfer Center, the Transfer Center Coordinator plays a vital role in facilitating patient transfers and ensuring seamless communication between referring hospitals and our healthcare providers. This position requires a strong understanding of clinical operations and the ability to provide administrative support effectively.
Key Responsibilities:
- Manage the intake process for incoming transfer requests and provide assistance to transferring facilities.
- Respond to inquiries and assist with patient care needs based on specific requirements.
- Generate Medical Record Numbers (MRN) and Financial Identification Numbers (FIN) for new patient admissions.
- Communicate efficiently with Intake RNs, referring facilities, and medical providers through verbal and electronic messaging.
- Facilitate warm handoffs between external referring providers and emergency department staff for incoming patients.
- Oversee future date direct admissions, ensuring clear communication with facilities and patients.
- Collaborate with central patient placement to ensure timely bed assignments.
- Act as a liaison for interfacility transfers, providing updates to sending facilities and documenting all interactions.
- Close out transfer requests upon completion and partner with patient placement for expedited bed assignments.
Additionally, the Coordinator will maintain a database for stroke transfers and generate monthly reports as required. Support for the Transfer Center RN will be provided as time allows.
Qualifications:
Applicants should possess a High School diploma or GED, along with a minimum of two years of experience in a healthcare setting. A solid understanding of medical terminology is essential for success in this role.
