POSITION SUMMARY

Under the direct supervision of the Care Coordination Outreach Supervisor, the Care Coordination Outreach Worker is responsible for facilitating the effective delivery of healthcare services to patients within Aunt Martha's Health and Wellness. This role includes conducting health risk assessments for assigned patients and scheduling appointments for individuals recently discharged from emergency or inpatient care. The Care Coordination Outreach Worker is expected to provide high-quality care, exceptional customer service, and maintain a safe and efficient care environment. This position requires superior customer service skills and the ability to manage multiple tasks and priorities while ensuring accuracy and attention to detail.

KEY RESPONSIBILITIES

  • Uphold patient confidentiality in accordance with established policies and procedures.
  • Exhibit ethical behavior consistently as per organizational standards.
  • Respond positively to both internal and external customer inquiries, utilizing the AIDET model to exceed expectations and deliver the highest quality service for optimal outcomes.
  • Become proficient in the electronic health record system, Athena, to accurately document patient information and manage outcome databases.
  • Conduct health risk screenings for all patients under care coordination.
  • Provide outreach services to active care coordinated patients through clinic, home, or hospital visits.
  • Deliver ongoing health education related to appropriate emergency department utilization and preventive services.
  • Answer designated phone calls courteously, route communications appropriately, respond to inquiries, and transcribe messages as needed.
  • Collaborate with health plans, specialists, hospitals, and physicians to ensure comprehensive patient care.
  • Attend all required trainings and meetings associated with the program.
  • Complete all mandatory Core Curriculum training(s) for this position.
  • Perform additional duties as assigned.

QUALIFICATIONS

To succeed in this role, candidates must demonstrate the ability to fulfill each essential duty satisfactorily. The following qualifications are necessary:

EDUCATION and EXPERIENCE

  • High School Diploma or Equivalent is required.
  • Proficiency in Microsoft Office applications is essential; strong data entry skills are necessary.

ADDITIONAL REQUIREMENTS

Possession of a valid driver's license.

Current auto insurance coverage.

Availability to work evenings, weekends, and scheduled holidays as required.
