ROLE SUMMARY
The Care Coordination Outreach Worker operates under the guidance of the Care Coordination Outreach Supervisor, engaging in essential functions that facilitate the effective delivery of healthcare services to patients within Aunt Martha's Health and Wellness. This role involves conducting health risk assessments for assigned patients and coordinating appointments for individuals recently discharged from hospital care. The Outreach Worker is committed to providing high-quality care and exceptional customer service while maintaining a safe and efficient care environment. This position requires strong multitasking abilities and meticulous attention to detail.
KEY RESPONSIBILITIES
- Uphold patient confidentiality in accordance with established policies and procedures.
- Exhibit ethical conduct consistently as per organizational standards.
- Address the needs of both internal and external customers positively, utilizing the AIDET model to exceed expectations and deliver superior service.
- Become proficient in the electronic health record system, Athena, to accurately document patient information and manage outcome databases.
- Conduct health risk screenings for all patients under care coordination.
- Provide outreach services to active care coordinated patients through clinic, home, or hospital visits.
- Deliver ongoing health education to patients regarding appropriate emergency department utilization and preventive services.
- Answer designated phone calls courteously, route communications appropriately, respond to inquiries, and transcribe messages as needed.
- Collaborate with health plans, specialists, hospitals, and physicians to ensure comprehensive patient care.
- Participate in all required training sessions and meetings.
- Complete all necessary Core Curriculum training for this role.
- Perform additional duties as assigned.
QUALIFICATIONS
To succeed in this role, candidates must demonstrate the ability to fulfill each essential duty effectively. The following qualifications are necessary:
EDUCATION AND EXPERIENCE
- High School Diploma or equivalent is required.
- Proficiency in Microsoft Office applications is essential, with strong data entry skills.
ADDITIONAL REQUIREMENTS
Possession of a valid driver's license and current auto insurance is required.
Availability to work evenings, weekends, and scheduled holidays as necessary.
