Position Summary

The role of the Care Concierge (CC) is pivotal in fulfilling Curana Health's mission to enhance the health, happiness, and dignity of our senior residents. The Care Concierge is often the primary point of contact for patients and their families, fostering strong relationships that are essential for a positive care experience. CCs undertake a variety of administrative responsibilities that enable their care teams to function effectively and efficiently. The quality of patient and family interactions is directly linked to the success of this role, as we prioritize exceptional care, experience, and communication.

Key Responsibilities

Facility Engagement
  • Organize and participate in educational and social events at facilities.
  • Serve as a liaison among key stakeholders, including nursing, sales, wellness, and leadership.
  • Remain attentive to facility needs and relay information back to the team.
Patient Engagement
  • Facilitate care coordination among specialists, therapy, home health, and other ancillary services.
  • Assist with front desk operations, including appointment scheduling and management.
  • Conduct monthly outreach to residents and families to evaluate needs and coordinate care in alignment with the provider's care plan.
Clinic Setup and Design
  • Support the setup and design of clinics according to Curana Health specifications.
  • Ensure timely ordering of necessary materials and supplies.
Clinical Functions
  • Welcome each patient, family member, and staff with a friendly demeanor.
  • Receive training to take vital signs, prepare patients for their appointments, and assist in the clinic.
  • Act as a backup for Medical Assistants within the Curana Health Clinic.
Qualifications
  • High School Diploma or equivalent required.
  • Experience in a senior living environment is highly preferred.
  • Outgoing personality with a passion for engaging with people.
  • Strong organizational and communication skills.
  • Background in concierge or hospitality services is advantageous.
  • Experience as a medical assistant, activities coordinator, social worker, or LPN is a plus.
  • Proficient in Microsoft Office applications.
  • Positive attitude and excellent interpersonal skills.
  • Dedicated to helping others and highly organized.
  • Willingness to learn and contribute to a meaningful mission.
