Role Summary
The Care Concierge (CC) is essential to Curana Health's mission of enhancing the health, happiness, and dignity of our senior residents. This role involves establishing meaningful relationships with patients and their families, ensuring they feel recognized and valued.

The CC will frequently be the primary point of contact for patients, fostering strong connections and providing exceptional service. This position includes various administrative responsibilities that enable the care team to function effectively and efficiently, directly impacting the patient and family experience.

Key Responsibilities
  • Facility Engagement:
    Coordinate and participate in educational and social events at the facility. Act as a liaison among key stakeholders, including nursing, sales, wellness, and leadership, while being responsive to the facility's needs.
  • Patient Engagement:
    Facilitate care coordination among specialists, therapy, home health, and other ancillary services. Assist with front desk operations, including appointment scheduling and management. Conduct monthly outreach to residents and families to assess needs and align care with the provider's plan.
  • Clinic Setup and Design:
    Support the setup and design of clinics according to Curana Health specifications. Ensure timely ordering of necessary materials and supplies.
  • Clinical Functions:
    Welcome each patient, family member, and staff member with a friendly demeanor. Receive training to take vital signs, prepare patients for their visits, and serve as a backup to the Medical Assistant in the clinic.
Qualifications
  • High School Diploma or equivalent.
  • Experience in the senior living environment is highly preferred.
  • Outgoing personality with a passion for engaging with people.
  • Strong organizational and communication skills.
  • Experience in concierge services or hospitality is advantageous.
  • Background as a medical assistant, activities coordinator, social worker, or LPN is a plus.
  • Proficiency in Microsoft Office.
  • Positive attitude and excellent interpersonal skills.
  • Commitment to helping others and a desire to contribute to a meaningful cause.
