Job Summary:

Coffee Regional Medical Center is seeking a highly skilled and compassionate Social Services Coordinator to join our team. As a key member of our healthcare team, the Social Services Coordinator will provide direct and indirect care to patients and their families, utilizing advanced social work assessment and intervention skills to facilitate optimal clinical, financial, and operational outcomes.

Key Responsibilities:

  • Assess patients' psychosocial needs and develop plans of care to address these needs.
  • Provide counseling and support to patients and their families to enhance their ability to cope with illness, hospitalization, and discharge.
  • Collaborate with the healthcare team to identify and coordinate referrals to community resources and services.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Document patient progress and outcomes in a timely and accurate manner.

Requirements:

  • Master's degree in Social Work (MSW) from an accredited program.
  • Minimum of three years of experience in a healthcare setting, preferably in a social work or case management role.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare team members.
  • Ability to think critically and analytically, with a strong understanding of social work principles and practices.
  • Basic computer literacy and proficiency in electronic medical records (EMRs).

Preferred Qualifications:

  • Experience working with patients with complex medical and psychosocial needs.
  • Knowledge of community resources and services, including entitlement programs and support groups.
  • Ability to speak multiple languages, with a focus on serving diverse patient populations.
