Job Summary

We are seeking a Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) to join our team in Douglas, GA. As a crisis support specialist, you will provide support to clients and staff working in our crisis support homes. Your primary responsibilities will include coordinating short-term out-of-home placements, developing discharge plans, and following up with individuals to ensure adherence to crisis-service-recommended support/services.

Responsibilities
  • Provide support to clients and staff in crisis support homes
  • Coordinate short-term out-of-home placements with stakeholders
  • Develop discharge plans detailing discontinuation from crisis support services
  • Follow up with individuals to ensure adherence to crisis-service-recommended support/services
  • Assist in providing information to the mobile crisis team
  • Facilitate referrals quickly to prevent escalation of crisis
  • Establish and maintain record keeping functions; integration with CIS system
  • Complete necessary documentation to facilitate reimbursement through primary funding sources
  • Develop and implement safety plans as appropriate
  • Report any suspected abuse, neglect, or exploitation to supervisor or department head
Qualifications
  • Must be a licensed LCSW, LPC, or LMFT
  • Valid CPR and First Aid Certification
  • Valid Driver's License and Auto insurance
  • Must be computer literate
  • Must be willing to flex schedule accordingly
  • Must maintain any applicable license, training, and/or certifications
  • Experience working with individuals with mental illness, emotional disorders, and substance-related disorders preferred
