We are seeking a compassionate and skilled Hospice Social Worker to join our team at IV Care of Middle GA. As a Hospice Social Worker, you will play a vital role in providing emotional, social, and spiritual support to patients and their families facing end-of-life care.
Key Responsibilities- Establish and implement goals and objectives for social services to meet the needs of patients and families.
- Provide emotional support and counseling to patients and their families.
- Assess and respond to the needs of patients and families in varied settings.
- Develop and maintain effective relationships with healthcare providers, patients, and families.
- Collaborate with the interdisciplinary team to ensure comprehensive care.
- Master's degree in Social Work from an accredited program.
- Licensure by the Georgia Board of Professional Counselors, Social Workers and Marriage & Family Therapists.
- Minimum of one year experience as a Social Worker in a healthcare setting.
- Knowledge of the Medicare Certified Hospice Program preferred.
- A dynamic and supportive work environment.
