Reports to the Personal Care Home Administrator

Role Objective

The role involves delivering direct residential assistance to the PCH Administrator, Shift Coordinators, and Direct Support Professionals. This position requires exceptional organizational capabilities and strong leadership potential. The ability to manage multiple tasks and handle high-pressure situations while upholding professionalism is crucial. Responsibilities include supporting the provision of services for residents, scheduling staff to maintain required ratios, and ensuring the health and safety of all residents.

Qualifications Required

A Bachelor's degree in Human Services, Business, or Social Services is preferred, along with a minimum of 2 years of relevant work experience or a combination of education and experience totaling 4 years. Experience in the Human Services sector is desirable, with a demonstrated commitment to working with adults with developmental disabilities/intellectual disabilities.

Applicants must be at least 21 years old, possess a valid Class C Georgia Driver's License, provide proof of vehicle insurance, and comply with company policy regarding motor vehicle record requirements.

Candidates must meet state regulations concerning a GBI and FBI criminal history record check, which includes fingerprinting, and must successfully complete a drug screening and tuberculosis screening.

Key Responsibilities

• Availability to work at least two evening, overnight, and weekend shifts per month to support continuous operations.

• Ensure accurate narcotic medication counts are conducted in accordance with established policies.

• Assist with medication administration as required.

• Familiarity with the Performance Development Protocol for medication errors and coaching Direct Support Professionals to minimize occurrences.

• Provide leadership in the absence of the PCH Administrator or Shift Coordinator.

• Responsible for staffing to meet regulatory ratios and staffing profiles, filling vacant shifts, and covering for call-outs.

• Develop a schedule that maximizes the availability of the PCH Supervisor to the Shift Coordinators and Direct Support Professionals.

• Conduct and/or oversee daily residential shift updates.

• Monitor and assess the physical condition and maintenance of residential facilities using the Residential Inspection checklist.

• Be on-call during evenings, weekends, and holidays as scheduled.

• Act as the Designated Person of Authority in the absence of the PCH Administrator.

• Ensure that daily documentation is entered into Point Click Care by residential staff.

• Ensure timely and accurate completion of Incident Reports within the shift they occur.

• Exhibit excellent interpersonal skills and the ability to collaborate effectively with all levels of internal management and staff.

• Ensure all staff fulfill their assigned duties with quality and provide excellent customer service to residents, families, and colleagues.

• Advanced computer skills with the capability to train others in system usage.

• Oversee meals in the dining area and during special events as needed.

• Supervise and assist community volunteers present on campus.

• Plan schedules for newly hired residential staff, including PRN and part-time positions.

• Attend Individual Support Plan meetings as assigned.

• Perform additional duties as necessary to support the mission of Annandale at Suwanee, Inc. in providing personal service and care to residents.

  6. Must be able to lift up to 25 pounds and/or move up to 75 pounds (with assistance).
  7. Specific vision abilities required include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
  8. Must be able to transport (drive) and assist residents in and out of vehicles.
  9. Must be trained to perform CPR/First Aid.
