Job Overview

We are seeking a skilled Clinical Pharmacist to join our team at Providence St Vincent Medical Center in Portland, OR. This full-time position offers the opportunity to provide high-quality pharmaceutical care and integrated pharmacy operational services.

About the Role

The successful candidate will be responsible for optimizing patient medication therapy outcomes and quality of life in collaboration with the healthcare team. A strong understanding of cardiovascular medications and treatment options is essential.

Estimated Salary: $87,427 - $144,819 per annum, depending on experience.

Responsibilities
  • Provide expert pharmaceutical care to patients with cardiovascular conditions.
  • Collaborate with healthcare teams to develop and implement individualized medication plans.
  • Stay up-to-date with the latest research and guidelines on cardiovascular medications.
  • Participate in ongoing quality improvement initiatives to ensure the highest standards of care.
Requirements
  • PharmD or Bachelor's Degree in Pharmacy.
  • Oregon Pharmacist License (upon hire).
  • Acute care hospital pharmacy experience (1 year or equivalent).
  • Completion of ASHP-accredited PGY1 residency (preferred).
