Job Title: Care Transition Specialist

We are seeking an experienced and skilled Care Transition Specialist to join our team at Option Care Health.

Salary Range: $73,916.54-$123,186.89 per annum

About the Role: As a Care Transition Specialist, you will play a critical role in ensuring seamless transitions for patients from hospital to home or alternate site infusion settings.

Responsibilities:

  • Establish and maintain strong relationships with referral sources and internal partners to ensure quality of service and effective treatment plans.
  • Use sales techniques to educate patients, families, and facility staff about Option Care services and products.
  • Conduct assessments of patients selected by hospitals to ensure viability for alternative site care.
  • Provide hands-on education to patients and caregivers to promote timely discharge and therapy independence.
  • Collaborate with account managers and regional sales directors to create and execute area business and growth plans.
  • Maintain confidentiality of patient and proprietary information.

Requirements:

  • Licensed Registered Nurse (RN) or licensed Pharmacist in the state of practice.
  • At least 2 years of experience in the healthcare industry.
  • Experience establishing and maintaining relationships with individuals at all levels of the organization and with vendors.
  • Ability to plan, organize, and make presentations.
