About the Role:

  • Job Type: Full-time, 7:30am - 4:30pm, Monday to Friday (with every other weekend rotation)
  • Duration: 6 months with possible extension
  • Location: [Undisclosed]

Responsibilities:

  • Round on patients in the hospital setting
  • Provide follow-up care in the home health environment
  • Work collaboratively with healthcare teams to ensure high-quality patient care

Requirements:

  • 5+ years of experience in acute, ambulatory, oncology, or geriatrics settings
  • Demonstrated expertise in clinical care and patient management
  • Strong communication and interpersonal skills
