Job Title: Social Services Assistant

We are seeking a compassionate and detail-oriented Social Services Assistant to join our team at Miracle Mile Healthcare Center. As a key member of our care team, you will play a vital role in ensuring the social, emotional, and psychological well-being of our residents.

Responsibilities:
  • Assist in conducting interviews and gathering information from residents and families to develop psychosocial histories, assessments, and care plans.
  • Support residents and families during the transition and adjustment to placement in our facility.
  • Determine the legal status of residents upon admission and ensure that their wishes regarding treatment are implemented in accordance with our company policy.
  • Inform residents and families of their personal and property rights.
  • Provide input on the social, psychological, and emotional needs of residents and families to supervisors.
  • Ensure residents receive necessary evaluations for visual, audio, mental health, and dental needs, and follow up as necessary.
  • Arrange residents' appointments, including transportation.
Requirements:
  • Previous experience working with geriatric populations or in long-term care settings is strongly preferred.
  • A high school diploma or equivalent is required; an associate's or bachelor's degree is preferred.
  • Ability to interact effectively and professionally with residents, families, and staff.
  • Must be able to relate to and work with individuals with diverse needs and backgrounds.
  • Willingness to work beyond normal working hours, weekends, and holidays as needed.
  • Sufficient computer skills, including proficiency in Microsoft Office and Outlook.
