Candidate Profile

The ideal Registered Nurse will possess the following qualifications:

  • Education: Must hold an active Registered Nursing license in New York State, with a Bachelor of Science in Nursing (BSN) required.
  • Experience: A minimum of 5 years of clinical experience is essential, with a preference for candidates who have at least 3 years of experience in care management within health plans, home health, or hospice settings.
  • Innovative Mindset: The RN Advocate is expected to address longstanding disparities in healthcare delivery for marginalized populations, demonstrating the ability to work autonomously and drive meaningful change.
  • Communication Skills: Strong verbal and written communication abilities are necessary, with a talent for presenting complex concepts in an understandable manner.
  • Relationship Building: Proven capability to cultivate and maintain effective relationships with patients, community leaders, and external stakeholders.
  • Team Spirit: A commitment to ethical practices, teamwork, and a desire for personal and professional growth in a dynamic environment.

Role Responsibilities

The RN Case Manager will report to the Director of Clinical Operations and will be responsible for strategic planning, organizational judgment, and evidence-based analysis to meet CINQCARE's objectives. The candidate should embody CINQCARE's core values: Trust, Empathy, Commitment, Humility, Creativity, and Community Orientation.

The RN will undertake the following tasks:

  • Collaborate with an interdisciplinary team to ensure optimal outcomes for CINQCARE's members facing chronic health challenges.
  • Create care pathway templates driven by alerts, tailored to specific conditions and risk levels.
  • Utilize these templates to formulate Member Action Plans based on chronic conditions.
  • Implement Remote Patient Monitoring and Patient Self-Reporting for individuals with high-risk chronic conditions.
  • Conduct assessments through in-home visits or telehealth as required.
  • Respond promptly to member alerts, maintaining active communication with members and caregivers via text and phone.
  • Coordinate care for CINQCARE's members, including navigation through chronic disease management and interdisciplinary collaboration.
  • Engage members in proactive health management, including medication adherence and follow-up appointments.
  • Adhere to evidence-based guidelines to close care gaps and facilitate access to in-network services.
  • Utilize electronic medical records for care coordination and ensure compliance with documentation policies.
  • Participate in team-based discussions to contribute to program design and improvement initiatives.
  • Reassess Member Action Plans post-discharge to ensure continuity of care.
  • Adapt priorities effectively to manage patient care loads.
  • Collaborate daily with the interdisciplinary team to discuss care planning and facilitation.
  • Perform additional job-related duties as assigned.

Leadership and Strategy

The RN Advocate will take a lead role in defining and executing strategies that create value within the clinical practice, including collaboration with team members to design and implement these strategies effectively.

Knowledge and Culture

The RN Advocate will ensure a cohesive clinical offering by working closely with other divisions, fostering a collaborative, safe, and inclusive work environment for the clinical team.

Work Environment

This position entails both in-home and office-based responsibilities, requiring frequent travel for home visits and interactions with various healthcare facilities. The role demands effective communication skills, the ability to manage multiple tasks, and the capacity to handle physical demands such as lifting and using office equipment.
