Candidate Profile

The ideal Registered Nurse (RN) should possess the following qualifications:

  • Education: Must hold an active Registered Nursing license in New York State, with a Bachelor of Science in Nursing (BSN) required.
  • Experience: A minimum of 5 years of relevant clinical experience is essential, with a preference for candidates having over 3 years in care management within health plans, home health, or hospice settings.
  • Innovative Mindset: CINQCARE seeks individuals who can identify and address longstanding gaps in care delivery for underserved populations. The RN Advocate should demonstrate the ability to work autonomously and drive positive change.
  • Communication Skills: Exceptional verbal and written communication abilities are required, along with the capacity to present ideas clearly and effectively.
  • Relationship Building: Proven ability to cultivate and maintain strong relationships with patients, community leaders, and external stakeholders.
  • Team Culture: Strong ethical standards, sound judgment, and a collaborative spirit are essential for success in this dynamic environment.

Role Responsibilities

The RN Case Manager will report to the Director of Clinical Operations and will be responsible for strategic planning, sound judgment, and evidence-based analysis to meet CINQCARE's objectives. The RN should embody CINQCARE's core values: Trust, Empathy, Commitment, Humility, Creativity, and Community Focus.

The RN will undertake the following tasks:

  • Collaborate with an interdisciplinary team to ensure optimal outcomes for members and families managing chronic conditions.
  • Develop care pathway templates driven by alerts, tailored to specific conditions and risk levels.
  • Create Member Action Plans utilizing care pathway templates for chronic conditions.
  • Implement Remote Patient Monitoring and Patient Self-Reporting for high-risk chronic conditions.
  • Conduct assessments via in-home visits or telehealth as necessary.
  • Respond promptly to member alerts and maintain active communication with members and caregivers through various channels.
  • Coordinate care for CINQCARE members, facilitating navigation through chronic disease management and interdisciplinary collaboration.
  • Engage patients in proactive health management, including medication adherence and follow-up appointments.
  • Utilize electronic medical records for care coordination and ensure compliance with established policies.
  • Participate in team-based discussions to enhance program design and development based on field insights.
  • Reassess Member Action Plans post-discharge to ensure continuity of care.
  • Adapt priorities effectively to manage patient care loads.
  • Collaborate daily with the interdisciplinary team to discuss care planning and facilitation.
  • Perform additional job-related duties as assigned.

Leadership and Strategy

The RN Advocate will take the lead in defining and executing strategies that create value within the clinical practice. This includes collaborating with team members to design and implement effective solutions aimed at achieving desired outcomes.

Work Environment

This role involves both in-home and office-based work, requiring frequent travel for home visits and interactions with healthcare facilities and community partners. The position necessitates the ability to travel in various weather conditions and communicate effectively with diverse individuals.

Physical requirements include the ability to lift up to 30 lbs occasionally, maintain good manual dexterity for office equipment, and engage in prolonged periods of sitting and communication through various devices.
