Job Summary

We are seeking a highly motivated and compassionate Community Health Worker to join our team. As a Community Health Worker, you will play a critical role in connecting our vulnerable population with their healthcare providers, conducting outreach to re-engage at-risk patients, and working to retain them in care to achieve desirable health outcomes.

Key Responsibilities
  • Conduct extensive outreach to re-engage at-risk patients and work to retain them in care
  • Collaborate with healthcare providers to ensure seamless patient care
  • Develop and maintain relationships with community partners to enhance patient engagement
  • Provide high-quality, patient-facing services to enhance the reputation of AHF
  • Advocate for AHF's top initiatives and ensure alignment with the organization's core values and mission
Requirements

To be successful in this role, you will need:

  • A high school diploma or equivalent required; Associate/Bachelor's degree in Public Health or related field preferred
  • Three years of related experience and/or training; or equivalent combination of education and experience
