We are seeking a highly skilled and compassionate Home Health Care Nurse to join our team at All Ways Caring HomeCare. As a key member of our care team, you will provide high-quality, patient-centered care to individuals in their homes, promoting independence, dignity, and comfort.
Key Responsibilities- Provide skilled nursing care and treatment to clients in their homes, as per physician orders and care plans.
- Accurately document nursing actions, client health status, and communicate with family members and physicians.
- Develop and implement individualized care plans, focusing on health promotion, disease prevention, and management.
- Conduct regular nursing assessments, identify potential health risks, and implement interventions to prevent complications.
- Administer medications, manage medication regimens, and educate clients on proper medication use.
- Provide health education and training to clients and their families, promoting self-care and independence.
- Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure comprehensive care.
- Maintain accurate and detailed records, adhering to regulatory standards and company policies.
- Participate in ongoing education and training to enhance clinical skills and knowledge.
- Degree in Nursing from an accredited institution.
- Current and unrestricted license to practice nursing in the state of operation.
- One or more years of nursing experience, preferably in home health care or a related field.
- Basic clinical skills, including medication administration and management.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work independently and as part of a team, with a keen attention to detail and a commitment to quality care.
